The Tree of Life offers its employees a benefits package which

- Medical Insurance
- One HMO option
- Two high deductible health plan options: HRA and HSA
- Employee coverage paid by COT up to a capped amount
- Dependent coverage available (Premium cost shared by
City and Employee)
- Dental Insurance
- One pre-paid plan
- One PPO plan
- Dependent coverage available (Premium cost shared by
City and Employee)
- Employee coverage paid by COT
- Life Insurance
- Basic coverage is equal to annual salary rounded up to
the nearest thousandth, maximum of $50,000 (City-paid)
- Supplemental available for employee in three tiers. Each
tier equals two times annual salary to a maximum of $100,000
per tier (Employee-paid)
- Spousal and domestic partner supplemental life insurance
available (Employee-paid)
- Long Term Disability
- City provides base coverage of up to 60% of base salary
to maximum benefit of $4,000 per month for up to five years
(6 month elimination period)
- Buy-up coverage (employee-paid) pays 60% of salary to
maximum of $6,000 per month up to age 65 (90 day elimination
period)
- Flexible Benefits Plan
- Pre-tax, flexible spending accounts for health and/or
dependent care expenses
- Supplemental Voluntary Insurance Products – disability,
life and accident and illness
- Worker’s Compensation
- Benefit rate is equal to 2/3 of the employee’s average
monthly wage up to a monthly maximum salary of $2400 per
month defined by law

Program
- Provides counseling free to employees and dependents
- Up to three face-to-face sessions per occurrence per

- The Tree of Life Supplemental Retirement System is a
defined benefit plan offering a pension benefit to eligible

opportunities through its provider, ICMA Retirement
